We bill most large insurance carriers as a courtesy for our patients; however, we always suggest you check with your insurance company to verify that our physicians are participating providers in your plan, please see the list of major insurance carriers that we accept, or call us to verify that your insurance is one we bill. When making your first appointment, please have your insurance information available to you. As with any insurance, you may be responsible for deductible and co-insurance amounts at the time of your appointment. Should you have any questions regarding billing and insurance, or if you do not have insurance, please contact our office at (985) 446-2680.
Patient Financial Responsibility and Payment Policy
It is our office policy that payment will be collected for patient deductibles, co-insurance and co-payment at the time of service.
***NOTE*** Although the physicians of Family Doctor Clinic may treat established patients covered by your insurance carrier, please contact us to determine if the physicians are still accepting new patients with your coverage.
